Mono Slab® EZ Form allows you to form, insulate, and back-fill all before you pour and place concrete, removing several steps in the forming and insulating process.
Product Description
Mono Slab® EZ Form is a patented form for shallow foundations. Mono Slab® EZ Form allows you to form and back-fill all before concrete placement. Mono Slab® EZ Form is simple, easy to use, and provides a superior process compared to traditional forming methods. Mono Slab® complies with IRC, IBC and IECC code requirements and is designed in accordance with ASCE 32.

Preparation
Dig and prep the site to accommodate for footer and width of the form. Typically, 5 to 6 feet wide is enough. Square and level footer and set a desired elevation. Minimal Excavation is required.
Installation
Prep footer to the desired elevation of footer grade. Next, begin setting forms, starting at a corner. To connect the lengths of Mono Slab® EZ Form, simply place a standard 2 by 8 in the allowed slot, staggering the 2 X 8 at the seam. Check level and square as you set each form.Stake each form with a wood stake to hold in place. After the forms, have been connected and staked, lightly back-fill against the outside of the form. This creates the downward pressure to hold the form in place. The forming process is complete.
